Job summary

Elara Caring in the Worcester area is seeking a Registered Nurse Behavioral Health Visits to provide patient‑centered nursing care in home settings. You will assess, plan, and implement the Interdisciplinary Plan of Care, coordinate with the Clinical Manager, and deliver 1:1 care that supports quality outcomes and patient independence.

Requirements include a current, unrestricted RN license, a high school diploma or GED plus a nursing degree, and the ability to travel within the branch area.

Qualifications

• Current, unrestricted state RN license.
• High School Diploma or GED with nursing degree (ADN/BSN).
• 1+ years in clinical care; home health experience preferred.
• Ability to travel within branch area and lift up to 50 pounds.

Responsibilities

• Assess caseload and plan care within Interdisciplinary Team.
• Coordinate and implement the plan of care for behavioral health patients.
• Provide 1:1 nursing visits and patient education.
• Document care accurately and communicate changes to supervisors.
