Lenape Valley Foundation in Doylestown, PA seeks a full-time Behavioral Health Follow-Up Specialist to conduct outreach after crisis or substance use services. You will help connect individuals with treatment, resources, and ongoing supports, and assist with 988 Suicide & Crisis Lifeline calls as needed.

The role requires a Bachelor's degree in a related field with two years of behavioral health experience, strong communication skills, and proficiency with electronic health records.

Qualificações

• Bachelor's degree in psychology, social work, counseling, sociology, nursing, human services, criminal justice, education, or a related field with two years of behavioral health direct care experience.
• Experience working with individuals experiencing mental health, substance use, or co-occurring challenges.
• Strong interpersonal, communication, and organizational skills.

Responsabilidades

• Conduct follow-up outreach to individuals after crisis or substance use services to support connection with treatment and resources.
• Encourage engagement with behavioral health and community services.
• Assist individuals in identifying and addressing barriers to treatment.
• Provide information regarding available resources and supports.
• Document all contacts and outreach activities in the electronic health record.
