University Behavioral Health of Denton is seeking a Discharge Planner to coordinate discharge planning and ensure continuity of care for patients after hospitalization. This role focuses on timely communication and collaboration with care teams to develop effective post-discharge plans.

The position emphasizes coordinating with referral sources, social workers, and nursing staff to minimize duplication and ensure smooth transitions.

Qualifications

• Bachelor's degree in Social Work or a clinical related mental health field preferred.

Responsibilities

• Plan and coordinate discharge planning to meet post-hospitalization needs and ensure continuity of care.
• Communicate with referral sources within 24 hours of admission and, with care center/social workers, identify outside professionals to complete discharge plans and minimize duplication.
• Identify needs of referral sources and professionals during hospitalization.
• Oversee ongoing communication of treatment progress, discharge plans, and timing of discharge to outside parties and transfer required documents.
