Job summary

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field required.
• Master’s degree preferred for advancement or specialized roles.
• Experience or coursework in crisis care, case management, or care coordination is valued.

Responsibilities

• Coordinate discharge planning from admission through post‑discharge follow‑up.
• Develop individualized discharge and safety plans reflecting clinical needs and client preferences.
