Job summary

Wellfound Behavioral Health Hospital in Tacoma, WA seeks a full-time Discharge Planner to join the Inpatient Team. Monday–Friday, 8:00 am–4:30 pm, on-site role integrating patient care and discharge planning.

The Discharge Planner works with the treatment team to address social determinants, arrange post-discharge resources, and document safe discharge plans from admission onward; requires BLS and a behavioral science degree or related experience.

Qualifications

• Bachelor's degree in behavioral science or related field (experience considered in lieu of degree).
• One year of related work experience in the mental health field strongly that demonstrates attainment of the requisite job knowledge skills/abilities.
• Basic Life Support (BLS) certification through American Heart Association or American Red Cross required to start.

Responsibilities

• Collaborate with patient and interdisciplinary team to develop a treatment plan.
• Assess and screen patients for resource eligibility and schedule post-discharge appointments.
• Assist in obtaining pre-authorizations/pre-certifications, insurance eligibility and authorizations for medications, hospital stay, community needs as needed to support the patient during hospitalization and upon discharge.
• Attend treatment team daily and coordinate the patient care with other interdisciplinary treatment team members.
• Communicate effectively with treatment team members and others regarding patient needs, and current issues.
• Assist patients with transportation needs, if needed, for discharge after vetting out any potential options.
• Support patient through their hospital stay through weekly outreach to identified outpatient supports regarding discharge-related planning and updates.
• Identify resources available to meet patient needs after discharge.
• Facilitate group education for patients regarding how to access community resources after discharge.
• Assist in monitoring and directing the milieu as requested by treatment team members.
