Job summary

Telecare in Olympia, WA is seeking a Discharge Planner to provide discharge planning services to members and families, coordinating with multiple agencies and services to ensure effective transitions.

You will collaborate with the Conservator’s Office, eligibility workers, Public Defender, Public Guardian, Court Services and County Mental Health Services, bringing two years of social work or related experience and current CPR/CPI/First Aid certifications.

Qualifications

• Two years of professional experience in Social Work, Nursing, or related field.
• Must be 18 years of age or older.
• CPR, CPI, and First Aid certifications required or within 60 days of employment.

Responsibilities

• Coordinate discharge planning services for members and families.
• Collaborate with Conservator’s Office, eligibility workers, Public Defender, Public Guardian, Court Services, and mental health systems of care.
• Engage with County Mental Health Services and related entities to ensure smooth transitions.
