Job summary

Valley Cities Counseling and Consultation is seeking a Discharge Planner for Recovery Place Inpatient Services in Washington. The role focuses on connecting residents to SUD/MH services and other supports after discharge, using a harm-reduction approach within a multidisciplinary team.

Responsibilities include coordinating access to services, liaising with care providers, and supporting residents in achieving individualized discharge goals.

Qualifications

• High School diploma required; proof upon hire.
• Register as Agency Affiliated Counselor within 30 days of hire.
• Understanding of addiction, withdrawal, and treatment resources.

Responsibilities

• Provide support and resources to residents to help them meet goals in treatment plans.
• Link residents to community-based services (primary care, housing, mental health, benefits).
• Liaise with hospitals, clinics, and managed care systems.
• Maintain client records per WAC, Medicaid, and contracts.
• Mediate or de-escalate resident conflicts as needed.
• Assist residents with safety planning.
