Job summary

Highland Springs, part of Lifepoint Health, seeks a full-time Discharge Coordinator for the Day Shift to support behavioral health patients. The role focuses on aftercare coordination, treatment planning involvement, and resource linkage to ensure seamless transitions after discharge.

The incumbent will work with patients and families, coordinating with providers and community resources to facilitate ongoing care and recovery, in a hospital setting.

Qualifications

• Bachelor's Degree in Social Work, Counseling, or Psychology.
• Three years of clinical experience in an inpatient behavioral health setting preferred.
• CPR certification and Crisis Prevention Training (CPI) within 30 days.

Responsibilities

• Provides aftercare coordination for patients needing ongoing medical/psychosocial care after discharge.
• Creates and maintains up-to-date aftercare coordination community resources.
• Participates in treatment planning by scheduling aftercare appointments identified by the patient’s provider and therapist.
• Provides resource information to patients and families for future care needs.
• Communicates proactively with clients, families, aftercare providers, and referral sources.
