Mercy Behavioral Health Hospital, in partnership with Lifepoint Health, seeks a Discharge Coordinator to support behavioral health patients as they transition from inpatient care.

You will coordinate aftercare services, build resources, and participate in treatment planning, ensuring smooth referrals and clear communication with patients, families, and providers throughout the post-discharge process. This full-time day shift role emphasizes compassionate, patient-centered care.

Competenze

• Bachelor's Degree in Social Work, Counseling, or Psychology, or three years of clinical inpatient behavioral health experience.
• CPR certification and CPI within 30 days of hire.
• Experience with psychiatric and chemical dependency patients.
• Unencumbered clinical license if eligible per state regulations preferred.

Mansioni

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
• Creates and maintains up to date aftercare coordination community resources.
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
• Provides resource information to patients and families as appropriate for future care needs.
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.
