Job summary

Highland Springs, part of Lifepoint Health, is seeking a Discharge Coordinator to join our behavioral health team in Highland Hills, OH. In this full-time role you will coordinate aftercare and connect patients with community resources to support ongoing medical and psychosocial care after discharge.

You will participate in treatment planning and ensure timely aftercare appointments, while communicating with patients, families, providers and referral sources to facilitate smooth transitions of

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology and clinical experience.
• Experience with psychiatric and chemical dependency patients.
• CPR certification and CPI within 30 days.

Responsibilities

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
• Creates and maintains up to date aftercare coordination community resources.
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
• Provides resource information to patients and families as appropriate for future care needs.
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.

Education

Bachelor's Degree in Social Work, Counseling, Psychology

Three+ years of clinical experience in inpatient behavioral health

Unencumbered clinical license if eligible/needed per state regulations

CPR certification and CPI within 30 days
