Job summary

Lifepoint Health’s Highland Springs campus seeks a Discharge Coordinator for behavioral health to ensure smooth transitions and ongoing care after discharge. The role emphasizes coordination with providers, patients, and families to arrange aftercare services and appointments.

The position requires a clinical background in inpatient behavioral health, CPR and CPI within 30 days, and a licensed or eligible credential. Comprehensive benefits and development opportunities are provided.

Qualifications

• Bachelor's Degree in Social Work, Counseling or Psychology or at least three years of clinical experience in inpatient behavioral health.
• CPR certification and CPI within 30 days.
• Unencumbered clinical license if eligible/needed per state regulations preferred.
• Prior experience with psychiatric and chemical dependency patients.

Responsibilities

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
• Creates and maintains up to date aftercare coordination community resources
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist
• Provides resource information to patients and families as appropriate for future care needs
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients’ treatment.
