Qualifications

• Bachelor’s degree in Social Work or related behavioral health field OR Associates degree with at least 2 years of experience in an acute or transitional outpatient setting.
• The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
• Master’s degree in behavioral health or related field is preferred.

Responsibilities

• Conducts a thorough, accurate, clinically sound Psychosocial assessment, diagnosis, and LOCUS score with the individual in a timely manner.
• Educates and informs patients of consents for treatment, communication, and compliance.
• Gather collateral information from the individual, their clinic, other providers, etc.
• Collaborates with the Treatment Team to develop and implement an individualized person-centered treatment plan.
• Actively coordinates care to ensure services are coordinated with facility staff and address gaps at discharge.
• Collaborate with Interdisciplinary treatment team to engage, monitor, and communicate with the individual.
• Document all services and individual activities in the medical record.

Education

Bachelor’s degree in Social Work or related behavioral health field

Master’s degree in behavioral health or related field
