Optima Medical is an Arizona-based medical group with 23 locations and 90 providers serving over 120,000 patients statewide. Our mission is to improve quality of life in Arizona through personalized healthcare, focusing on preventing leading causes of death. We offer a full spectrum of services including behavioral health, cardiology, allergy testing, in-house lab testing, imaging, and chronic disease management. We are seeking a Behavioral Health Care Coordinator to work with our care team, collaborating with Primary Care providers to facilitate care coordination and counseling for patients with mental illness.

About the Collaborative Care Model (CoCM)

CoCM is a team-based approach to mental health involving primary care providers, behavioral health specialists, and psychiatric providers to deliver integrated care. Its goals are to improve access to mental health services, enhance patient outcomes, and provide holistic, coordinated care.

Responsibilities
  1. Conduct initial assessments of patients' mental health status and treatment needs, including screening for depression and anxiety.
  2. Collaborate with healthcare team members to develop and implement evidence-based treatment plans.
  3. Provide brief interventions, counseling, and psychoeducation to support mental health and well-being.
  4. Monitor patients' progress and treatment adherence through follow-ups and assessments.
  5. Coordinate care transitions and referrals to mental health services or community resources.
  6. Educate patients and families about mental health conditions, treatment options, and self-care strategies.
  7. Document patient encounters, treatment plans, and outcomes in electronic health records (EHR).
  8. Participate in team meetings, case reviews, and quality improvement initiatives.
  9. Collaborate with external mental health providers and community organizations to address psychosocial needs.
  10. Maintain patient confidentiality and uphold professional boundaries in accordance with ethical and legal standards.
Requirements
  1. Bachelor's Degree in Sociology or Psychology.
  2. Proven experience as a Behavioral Health Specialist or Clinical Social Worker in a clinical setting.
  3. Ability to work as part of a multidisciplinary team.
  4. Knowledge of health and patient care regulations.
  5. Excellent communication skills.
  6. Strong ethics and team spirit with a positive attitude.
  7. Willingness to continue gaining knowledge and clinical experience.
  8. Must reside in AZ.
