Job summary

COPE Community Services in Tucson, AZ is seeking a Community Support Liaison to coordinate care between the Crisis Response Center and outpatient teams. The role prioritizes urgent enrollments for COPE while patients are hospitalized to ensure timely connections to services.

You will provide flexible coverage across COPE Residential sites as part of the CSL Team, supporting program needs and patient wellbeing through proactive communication.

Qualifications

• Able to obtain and maintain Level I Fingerprint Clearance Card.
• Must pass pre-employment drug screen and background check.

Responsibilities

• Serve as liaison between the Crisis Response Center (CRC) and Outpatient Treatment Teams.
• Facilitate communication, coordination, continuity of care, and discharge planning.
• Complete urgent enrollments for COPE while patients are hospitalized and ensure timely connection to services.
• Provide flexible coverage across COPE Residential sites as part of the CSL Team.

Education

Bachelor’s or Master’s degree

Associate’s degree

High School Diploma or GED + Peer Support Specialist certification

High School Diploma or GED + Medical Assistant certification

High School Diploma or GED + 3 years behavioral health work experience
