Qualifikationen

• Must be able to obtain and maintain a Level I Fingerprint Clearance Card throughout employment.
• Must be able to pass a pre-employment drug screen and background check.
• Must be 21 years of age with AZ driver’s license and clean driving record.
• At time of offer, submission of 36 month driving record from DMV is required.

Aufgaben

• The Community Support Liaison serves as a liaison between the Crisis Response Center and Outpatient Treatment Teams to facilitate communication and coordination.
• Completes urgent enrollments for COPE while patients are hospitalized to ensure timely connection to services.
• Provides flexible coverage across COPE Residential sites as part of the CSL Team and supports discharge planning.

Ausbildung

Level I Fingerprint Clearance Card

Bachelor's or master's degree, no experience necessary

Associate's degree + 1 year behavioral health work experience

High School Diploma or GED + Peer Support Specialist certification + 2 years behavioral health work experience

High School Diploma or GED + Medical Assistant certification + 2 years behavioral health work experience

High School Diploma or GED + 3 years behavioral health work experience
