COPE Community Services in Tucson, AZ is seeking a Community Support Liaison to coordinate care between the Crisis Response Center and outpatient teams. The role prioritizes urgent enrollments for COPE while patients are hospitalized to ensure timely connections to services.
You will provide flexible coverage across COPE Residential sites as part of the CSL Team, supporting program needs and patient wellbeing through proactive communication.

Salary Range: $18.50 - $18.50Hourly
The Community Support Liaison (CSL) serves as a liaison between the Crisis Response Center (CRC) and Outpatient Treatment Teams to facilitate communication, coordination, continuity of care, and discharge planning. The CSL completes urgent enrollments for COPE while patients are hospitalized, ensuring timely connection to appropriate services and supports. The CSL provides flexible coverage across COPE Residential sites as part of the CSL Team, offering assistance and support based on program and patient needs.
MINIMUM EDUCATION/EXPERIENCE QUALIFICATIONS:
Able to obtain and maintain throughout employment a Level I Fingerprint Clearance Card.
Plus one of the following:
Must be able to pass a pre-employment drug screen and background check.
Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.

Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
