Qualifications

• Master's degree in a relevant human services field.
• Must be issued an associate licensure and/or certification within 6 months.
• CPR and CPI training completed within 90 days of hire.
• Clinical experience preferred; case management background helpful.

Responsibilities

• Collaborate with guardians and community agencies to meet patient needs.
• Assess patient problems, needs and functioning level.
• Develop and write treatment plans based on diagnosis, age, and presentation.
• Provide treatment modalities aligned with current clinical standards.
• Counsel individuals, groups and families and address mental disorders with psychotherapy.
• Draft discharge plans and manage termination process.
• Maintain patient care documentation in the medical record accurately.
• Present cases at clinical team meetings in a clear, concise way.
• Complete documentation within facility timeframes and coordinate discharge.
