Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term, evidence-based counseling for patients with behavioral health needs. You will coordinate with primary care teams and a psychiatric consultant to develop care plans and referrals, ensuring timely and effective care.

The role requires a master's level degree in a mental health field and the highest clinical license, with preference for bilingual Spanish speakers.

Qualifications

• Master's degree in mental health counseling, social work, or marriage and family therapy
• Licensed (LCSW, LPC, or LMFT) at the highest clinical level
• Experience screening for mental health and/or substance use disorders
• Experience with assessment and treatment planning for mental health and/or substance use disorders
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral)
• US work authorization

Responsibilities

• Collaborate with care teams to identify patients for the Oak Street Behavioral Health Program
• Screen and assess patients for mental health and substance use disorders
• Develop care plans and coordinate referrals to psychiatric consultants or external resources
• Provide short-term counseling and evidence-based treatments
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth
• Develop relationships with primary care teams and communicate changes to care plans
• Accurate documentation of patient encounters in the EMR
• Participate in regional and organizational program improvement efforts
• Possibly cover multiple clinics as needed
• Other duties, as assigned
