Job summary

Easterseals Blake Foundation is seeking a Behavioral Health professional to coordinate comprehensive behavioral health services for adult and child members enrolled in public health programs. The role includes intakes, assessments, and providing therapy and support services within a team-focused environment.

The position requires strong clinical knowledge, ability to work in high-stress settings, and collaboration with diverse teams across multiple sites in Arizona.

Qualifications

• Must have strong clinical knowledge across the life span.
• Familiarity with DSM-5-TR for diagnoses.

Responsibilities

• Assess members’ behavioral health needs and eligibility.
• Provide intakes, assessments, and therapy/support services as needed.
• Coordinate communication between members and providers and arrange transport.
• Deliver rehabilitation and supported employment services.
• Maintain documentation and complete records per requirements.
