Qualifications

• High school diploma or GED with 1 year of behavioral health experience.
• Associate or Bachelor in Behavioral Health acceptable.
• Bilingual Spanish preferred.

Responsibilities

• Assess members' behavioral health needs and eligibility for services.
• Provide comprehensive intakes, assessments, and scheduling referrals.
• Coordinate communication between members and providers to ensure care coordination.
• Provide rehabilitation services including living skills training, counseling, and supported employment.
• Provide case management, personal assistance, family support, and transportation services.
• Conduct rapid response and/or intakes as needed.
• Perform crisis intervention and safety planning.
• Maintain documentation and reporting of services per requirements.
• Participate in professional development and team communications.
