Job summary

Easterseals Blake Foundation seeks a dedicated behavioral health professional to coordinate services for adults and children enrolled in public behavioral health programs. You will assess needs, conduct intakes, and collaborate with providers to ensure comprehensive care.

Responsibilities include scheduling, crisis intervention when needed, and maintaining detailed documentation while supporting clients through rehabilitation and community-based services.

Qualifications

• High School Diploma or GED required; 1 year of behavioral health or related experience, or an Associate’s/Bachelor’s degree in Behavioral Health.
• Bilingual Spanish preferred.
• Must pass criminal background check and maintain AZ DPS Level 1 fingerprint clearance card; driver’s license required.

Responsibilities

• Assess members’ behavioral health needs and eligibility for services; assist in developing and monitoring service plans.
• Provide intakes, assessments, and ongoing therapeutic/support services; schedule referred members for services.
• Coordinate with other providers and community agencies to ensure care continuity; arrange transportation and attend telemed appointments.
• Deliver rehabilitation services including living skills training, counseling, and supported employment.
• Provide case management, family support, peer support, respite care, and transportation services.
• Document and report member services per agency and funding requirements; maintain records.
