Job summary

Easterseals Blake Foundation seeks a Behavioral Health professional to coordinate services for adult and child members enrolled in public behavioral health programs. You will perform intakes, assessments, and clinically-driven support across care teams, ensuring timely access and continuity of care.

The role emphasizes collaboration with providers, care coordination, crisis intervention, and documentation adherence in a fast-paced environment.

Qualifications

• High School Diploma or GED plus 1 year behavioral health or related experience or an Associate’s/Bachelor’s in Behavioral Health.
• Bilingual (Spanish) preferred.
• Education/experience level determines BHT vs BHP position level.

Responsibilities

• Assess members' behavioral health needs and service eligibility.
• Provide intakes, assessments, and therapeutic/support services.
• Coordinate communication with providers and agencies to ensure care continuity.
• Deliver rehabilitation services including living skills, counseling, and supported employment.
• Provide case management, personal assistance, family and peer support, and transportation.
• Perform rapid responses, intakes, and crisis interventions.
• Maintain productivity, timelines, and documentation per policy.
