The position requires a High School Diploma or higher in Behavioral Health, bilingual (Spanish) preferred, valid AZ driver’s license, and willingness to travel for member visits.

Qualifications

• Must understand behavioral health concepts and care coordination.
• Ability to work with diverse populations and trauma-informed approaches.
• Ability to use computer systems for documentation.

Responsibilities

• Assesses members' behavioral health needs and eligibility for services.
• Provides intakes, assessments, and coordination of care with providers.
• Schedules appointments and arranges transportation as needed.
• Provides case management and supportive services to enrolled members.
• Maintains documentation per funding requirements.
