Arrowleaf in Vienna, IL is hiring a Case Manager for a full-time direct service role. The position reports to the program coordinator and focuses on engagement, care coordination, and discharge planning within regional behavioral health services.

Qualified candidates will have a high school diploma or GED, RSA eligibility, and aim to become a Mental Health Professional within 18 months. Prior case management experience is preferred.

Competenze

• High School Diploma or GED required.
• Qualifies as a Rehabilitative Services Associate (RSA).
• Must qualify as Mental Health Professional (MHP) within 18 months.
• Previous case management experience preferred.
• Knowledge of case management processes and local resources.

Mansioni

• Provide outpatient mental health case management and community support.
• Coordinate care and discharge planning for clients.
• Assist with transportation and life-skills coaching.
• Document progress on Individual Treatment Plans.
• Maintain network/referral contacts and engagements.
• Complete required direct service hours and forms monthly.
