• Bachelor's degree in an applicable human services field.
• Minimum one year of relevant mental health, developmental disability or substance abuse work experience.
• Equivalent combination of education, training and experience may apply.

Responsibilities

• Assess, monitor, link to services, provide supportive-level counseling, education, and advocacy.
• Focus on Permanent Supportive Housing to help clients maintain stable housing.
• May travel to Horizon sites and transport clients as needed.
