Arrowleaf is hiring a Case Manager (Community Support Team) in Vienna, Anna, or Cairo, IL. The role supports behavioral health services and care coordination for adults in the Southern Illinois region.

The candidate must have a Bachelor's Degree in Human Services or related field and qualify as a Mental Health Professional. Duties include case management, crisis response, and facilitating transitions in care, with a strong focus on community-based support.

Qualifications

• Bachelor’s Degree in Human Services or related field required and must qualify as a Mental Health Professional (MHP).
• Knowledge of behavioral and human development theories and practices.
• Excellent written and oral communication skills; conflict resolution abilities.

Responsabilités

• Provides comprehensive consumer care and casemanagement services including access to transportation, entitlements, medical and psychiatric care, housing, and other needs.
• Supports community-based activities to promote independent living and daily living skills.
• Collaborates with treatment team to set goals and monitor progress; conducts advocacy and recovery planning.
• Facilitates communication across agency staff to ensure continuity of care and timely follow-up.
• Assists with transitions in care and linkages for those entering or exiting higher levels of care.
• Teaches life-management skills and documents services and billing efficiently.
• Participates in meetings, supervision, and agency events; provides crisis intervention as needed.
