Responsibilities

• Provides crisis stabilization and peer-based support services off-site in clients' natural environments according to safety plans.
• Facilitates communication across agency staff and programs to ensure continuity of care for individuals in crisis.
• Provides timely follow-ups with individuals, families, and referral sources, and updates safety plans as needed.
• Serves as liaison between Arrowleaf and facilities to coordinate post-discharge plans and connect to counseling and recovery supports.
• Offers comprehensive case management to help individuals access transportation, entitlements, medical and housing resources to support stabilization.
• Performs outreach to those at risk of crisis to improve engagement and access to stabilization services.
• Maintains networking with other providers to expand resources and supports.
