Qualifications

• Requires master’s degree in social work or related field or equivalent experience.
• Requires 5 years clinical experience and licensure in social work, professional counseling or marriage and family.
• Board licensure in addictions for substance abuse clients may be required by state.
• Must have knowledge of care management, hospital and community resources; strong communication and time management skills.

Responsibilities

• Manages individual patients across the health care continuum to achieve optimal outcomes.
• Provides assessment, psychoeducation, and psychotherapy; coordinates care from admission to discharge.
• Leads process improvement activities for patient populations to improve outcomes.
• Evaluates medical necessity and appropriateness of care to optimize outcomes.
• Establishes collaboration with physicians, payers and health care team; communicates pertinent information for utilization and regulatory requirements.
• Leads discharge planning and ensures seamless care transitions.
• Educates health care team on case management concepts and integrates them into daily practice.
