Job summary

Community Bridges, Inc. in Arizona is seeking a Case Manager to deliver community and office-based case management, coordinating services and support for patients and families.

You will help clients access health care, financial assistance, employment, education, and social supports, while developing and monitoring individualized treatment plans and documenting progress. The role emphasizes recovery and care coordination with the team.

Qualifications

• Highschool diploma or GED required; Associates degree preferred.
• 1–3 years of GMH/SU population experience.
• 1–3 years of outreach and engagement experience.
• 1–3 years in a behavioral health setting preferred.
• Valid AZ Driver's License preferred; 39-month MVR preferred.
• Arizona Fingerprint Clearance card is required.

Responsibilities

• Provide case management in community and/or office setting.
• Assist patients in accessing health services, financial assistance, employment, education, and social supports.
• Develop and monitor individualized treatment plans and document services.
• Embrace the recovery philosophy and promote a safe recovery environment.
