Job summary

Milestone Treatment Center is seeking a compassionate Case Manager to support individuals in treatment for substance use and mental health conditions. You’ll assess needs, develop plans, and connect clients with housing, employment, insurance, and other real-world resources.

This role emphasizes problem-solving, advocacy, and collaboration with therapists and a multidisciplinary team to remove barriers to recovery and help clients progress toward stability in the community.

Qualifications

• Experience in case management or related behavioral health services.
• Active or pending LCADC or QMHP-A credential preferred.
• Ability to connect individuals with community resources.
• Strong interpersonal and communication skills.
• Ability to advocate while maintaining professional boundaries.
• Excellent organization and attention to detail.
• Ability to manage multiple clients and priorities.
• Comfortable working independently within a multidisciplinary team.
• Calm, solution-focused demeanor in challenging situations.
• Proficiency with Google Workspace and electronic documentation systems.

Responsibilities

• Build relationships with clients and provide ongoing support and advocacy.
• Assess client needs, challenges, strengths, and resources.
• Develop and monitor individualized case management plans.
• Connect clients with housing, employment, healthcare, and other services.
• Help navigate housing programs, insurance, and government assistance.
• Verify benefits and coordinate related activities.
• Provide support during crises and interventions.
• Facilitate groups or educational sessions.
• Advocate to remove barriers to recovery and community reintegration.
• Coordinate with providers, families, and community organizations.
• Collaborate with therapists and multidisciplinary team.
• Maintain case notes, referrals, and client records.
• Track progress and follow up on outstanding needs.
• Maintain confidentiality and professional boundaries.
• Follow HIPAA, 42 CFR Part 2 and Milestone requirements.
