Job summary

Milestone Treatment Center is seeking a compassionate Case Manager to support individuals in treatment for substance use and mental health conditions. You will identify needs, connect clients with resources, and remove barriers to recovery and long-term stability.

You will work with a multidisciplinary team, advocate for clients, and help navigate housing, insurance, employment, and other real-world challenges while maintaining confidentiality and professional boundaries.

Qualifications

• Experience in case management, social work, behavioral health, substance use treatment, mental health, crisis intervention, or human services.
• Active or pending LCADC or QMHP-A credential preferred.
• Experience connecting individuals with community resources and support services.
• Strong interpersonal and relationship-building skills.
• Excellent verbal and written communication skills.
• Ability to advocate effectively while maintaining appropriate professional boundaries.
• Strong organizational skills and attention to detail.
• Ability to manage multiple clients, priorities, and follow-up activities.
• Comfortable working independently while contributing to a multidisciplinary team.
• Ability to remain calm and solution-focused when clients encounter difficult situations.
• Proficiency with Google Workspace and comfort learning electronic documentation systems.

Responsibilities

• Build strong, professional relationships with clients and provide support, guidance, and advocacy.
• Assess each client's needs, challenges, strengths, and resources.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with resources related to housing, employment, healthcare, transportation, benefits, and other services.
• Help clients navigate housing programs, government assistance, insurance, and community services.
• Verify benefits and related coordination when required.
• Provide support during challenging or crisis situations.
• Facilitate groups or educational sessions as needed.
• Advocate for clients and remove barriers to treatment, recovery, and community reintegration.
• Coordinate with outside providers, families, and community organizations as appropriate.
• Collaborate with therapists, medical providers, counselors, and other team members.
• Maintain timely and accurate case notes, documentation, referrals, and client records.
• Track client progress and follow through on outstanding needs.
• Maintain professional boundaries and client confidentiality.
• Follow HIPAA, 42 CFR Part 2, regulatory, ethical, and Milestone requirements.
