Job summary

Milestone Treatment Center is seeking a compassionate Case Manager to support individuals receiving treatment for substance use and mental health conditions. This on-site, full-time role in New Jersey offers a $40,000–$50,000 annual salary and comprehensive benefits.

Qualifications

Experience in case management, social work, behavioral health, substance use treatment, mental health, crisis intervention, or human services is strongly preferred.

Responsibilities

Build strong, professional relationships with clients and serve as a source of support and advocacy.
Assess each client's needs, challenges, strengths, and resources.
Develop, implement, and monitor individualized case management plans.
Connect clients with housing, employment, healthcare, behavioral health, transportation, benefits and other services.
Navigate housing programs, government assistance, insurance, and community services.
