Job summary

Kingwood Pines Hospital is seeking a dedicated Case Manager to advocate for patient access to services during treatment team meetings and through physician contacts. The role includes participating in treatment team discussions and continued stay reviews as needed.

You will coordinate discharge planning with families and other service providers, and support timely communications with payors to ensure appropriate certification and stay length.

Qualifications

• High School Diploma is required.
• Minimum of one year of case management/inpatient psychiatric experience or comparable experience. Demonstrated proficiency in psychiatric care required.
• CPR and HWC Crisis Intervention training must be completed prior to beginning work (Can be obtained upon hire).

Responsibilities

• Provides clinical information to payors to establish length of stay and certified days.
• Conducts reviews and documentation per payor standards to maximize reimbursement; liaison for internal/external reviewers.
• Provides timely updates regarding patient status for admissions, reviews and discharges.
• Coordinates discharge plans with family, guardians and other appropriate service providers.
