Job summary

Vail Health is seeking a Behavioral Health Case Manager to deliver comprehensive case management across the community, focusing on discharge planning and coordination with behavioral health providers and community partners. The role emphasizes patient-centered, recovery-oriented wraparound care to enhance access to services and improve outcomes.

The position requires at least one year in human services or behavioral health, HIPAA knowledge, and a Colorado driver’s license.

Qualifications

• One year of experience in human services, behavioral health, or related field required.
• Understanding of HIPAA and privacy laws with specific understanding of Part 2/42 CFR restrictions.
• Understanding Colorado laws and regulations related to mental health practices and confidentiality preferred.

Responsibilities

• Provides comprehensive case management services across the community to ensure consistent and coordinated delivery of services for high-risk, high-acuity community members.
• Manages a comprehensive caseload of 32 billable hours per week.
• Maintains clinical documentation in alignment with local, internal, state, federal, and regulatory standards and compliance.
• Engages in comprehensive service plans and client assessments addressing clinical need and SDoH.
• Completes comprehensive discharge planning for patients discharging from higher levels of care.
• Supports care coordination with behavioral health and medical providers to improve access and continuity of care, including home visits as needed.
• Develops case management care plans and coordinates attending care coordination meetings with providers and community partners.
• Facilitates referrals and coordination with community resources for total health outcomes.
• Supports education, prevention, and skill development to improve client and family outcomes.
• Monitors patient outcomes and collaborates to improve quality of care delivery.
• Analyzes data to inform standards and prepares reports and statistics as required.
• Maintains confidentiality of data in accordance with policy and regulations.
