Qualifications

Responsibilities

• Provide care safely and coordinate services for patients and families during and after hospitalization.
• Initiate and participate in family conferences to determine psychosocial and discharge needs.
• Complete psychosocial assessments addressing social, psychological, emotional, and financial factors.
• Connect patients to community resources and assist with Medicaid/Medicare and waivers as appropriate.
• Advocate for patients and caregivers to access services and protect health and rights.
• Document all encounters within 24 hours and participate in interdisciplinary meetings.
