Job summary

Kingman Regional Medical Center is seeking a Case Manager to collaborate with physicians, nurses, social workers, and other departments to coordinate patient care and post-hospital services in Kingman, AZ. The role emphasizes discharge planning, utilization review, and ensuring safe transitions from hospital to home or another facility.

The Case Manager will engage patients and families, assess needs, coordinate with payers, and maintain comprehensive documentation while contributing to quality

Qualifications

• RN license in Arizona (active)
• BLS certification per American Heart Association or Red Cross
• Bachelor’s degree in health/behavioral/social sciences or nursing
• Graduate from an accredited school of nursing
• Knowledge of discharge planning and utilization management is preferred

Responsibilities

• Collaborate with physicians, nurses, social workers to improve care and discharge planning.
• Involve patient/family in evaluating and planning post-hospital services.
• Identify community resources to support post-discharge needs.
• Ensure discharge plan is implemented and education provided.
• Perform Utilization Review (UR) and determine medical necessity using MCG criteria.
• Communicate coverage determinations with payers and educate patients about benefits.
