Job summary

Y.A.P.A. Apartment Living Program Inc in Philadelphia, PA is seeking a Case Manager to develop and implement treatment plan goals with Members, partnering with a multidisciplinary team to support transitions to less structured living.

The role includes coordinating admissions, completing incident reports, weekly case management sessions, documentation, on-call support, and helping Members with daily living activities and health/safety planning. Full-time benefits available.

Qualifications

• High school diploma or equivalent with relevant field experience.
• Associates degree plus 2+ years in mental health or related area.
• Experience with Project Transition/ PCS Mental Health preferred.

Responsibilities

• Collaborate with admissions to support the admission and acclimation of new Members into the program setting.
• Complete Incident Reports in accordance with processes outlined by the Managed Care Organization (MCO), State and Project Transition/ PCS Mental Health Policy and Process.
• Collaborate with members of the multidisciplinary team to develop treatment plans for identified Members.
• Complete weekly individual Case Management sessions with Members and document in DAP format.
• Facilitate/ Co-facilitate all groups as assigned and complete documentation.
• Support with transporting Members to appointments per the directive of the Program Director.
• Support with observation of Member drug and alcohol screenings and document observations (if applicable).
• Participate in phone coaching/ crisis management process, which includes orienting Member and family to coaching process and responding to calls.
