Job summary

Y.A.P.A. Apartment Living Program Inc in Lansford, PA is seeking a Part Time Assignee to provide direct oversight of Member residential living arrangements, ensuring safety, cleanliness and a supportive recovery environment.

You will document Member self-administration of medications, conduct roving inspections, assist with medication and ADL support, complete incident reports, and help with group facilitation, supervision, and progress notes in the EHR (DAP format).

Qualifications

• High School Diploma or equivalent.
• Ability to maintain confidentiality.
• Must possess a valid, non-commercial driver's license with a clean driving record and ability to operate company vehicles in accordance with safety regulations.

Responsibilities

• Roving and inspection of residences and Member ADL support as needed.
• Support with facilities and medication management activities, including disaster drills, checklists and medication room checks.
• Implement and document Member interventions and progress relative to Member Treatment Plan as it pertains to medication and ADL compliance.
• Complete Incident Reports in accordance with Program Director's Policy and Process.
• Complete progress notes for interactions with Members and document in the Electronic Health Record (EHR) (DAP format).
• Facilitate/ co-facilitate all groups and complete documentation within 24 hours of facilitating.
• Support with transporting Members per the directive of the Program.
• Observe and document Member alcohol and drug screenings as assigned and record observations.
• Support Members with activities of daily living including daily living oversight, medication self-administration, socialization/communication skills, conflict resolution, and crisis supports.
• Assist with meal planning, menu development, food shopping and meal preparation.
• Encourage health and home safety practices and navigation of public transportation services.
• Attend supervision sessions and program meetings as scheduled by supervisor.
