Job summary

Connections Health Solutions in Harrisburg, PA is seeking a Case Manager to oversee care for youth and adults in crisis, facilitating admissions, treatment planning, and discharge planning. You will work with an interdisciplinary team to coordinate services, gather collateral, and ensure documentation is complete.

A Bachelor's degree in behavioral health or related field is required, with strong knowledge of Pennsylvania's behavioral health system.

Qualifications

• Bachelor's degree in behavioral health or related field or 4 years of behavioral health experience.
• Knowledge of the Pennsylvania State behavioral health system and resources.
• Must pass background checks: Child Abuse Clearance, FBI Background Check, Pennsylvania State Police Criminal Record Check.
• Company vaccination policy requires full vaccination or approved exemption before start date.
• Complete credentialing requirements including NPI, Medallion enrollment, Medicare enrollment and other credentials as required.

Responsibilities

• Admission & Consent: Facilitate admissions and explain consents for treatment.
• Treatment Planning: Develop and implement individualized treatment plans with the team.
• Collateral Gathering & Assessment: Collect collateral information for needs and discharge planning.
• Care Coordination: Coordinate care with facility staff and external providers.
• Discharge Planning: Monitor and communicate discharge plans with families.
• Documentation: Maintain accurate clinic records and obtain signatures as needed.
• Community & System Knowledge: Maintain knowledge of PA behavioral health resources and funding.
• Professional Development: Participate in ongoing training and competency assessments.
