Job summary

Horizon Health and Wellness, Inc. is seeking a Care Manager to coordinate client services with compassion and efficiency. You will manage a caseload, advocate for clients, and collaborate with a multidisciplinary team to ensure seamless care delivery.

Ideal candidates have a HS diploma or GED with Behavioral Health experience and may hold CPR/First Aid certification. Bilingual English/Spanish is a plus, and a valid driver’s license is expected.

• Care Manager must provide services to client caseload in a coordinated and effective manner.
• Caring and compassionate delivery of case management, advocacy and other supportive services.
