Qualifications

• Experience: minimum of 3 years of case management experience.

Responsibilities

• Act as liaison between therapist, care team, client and family to coordinate care.
• Arrange family visits, coordinate transportation and ensure client needs are met.
• Maintain clear communication between family, referrals source and Path Behavioral Healthcare.
• Attend court hearings as appropriate.
• Ensure documentation and billing adhere to policy and standards in the EHR system.
• Maintain compliance with charting, audits, and Phase System of Care requirements.
• Attend supervisory and team meetings; present case management reports.
• Facilitate individual and group meetings as assigned.
• Oversee ISP-related activities and crisis intervention planning.
