Path Behavioral Healthcare is seeking a dedicated Case Manager in Findlay, OH to coordinate and deliver comprehensive support services. You will assess, plan, implement, and monitor client care in collaboration with the treatment team to promote recovery and independence.

The role requires 3+ years of case management experience, strong communication skills, and proficiency with Electronic Health Records. Must meet HIPAA standards and work as part of a multidisciplinary team.

Competenze

• Must have High School Diploma or GED; Bachelor’s degree preferred.
• Minimum of 3 years of case management experience.
• Excellent verbal and written communication skills.
• Ability to work under stressful conditions and irregular hours.
• Experience with HIPAA compliance and confidentiality policies.

Mansioni

• Act as liaison between the therapist, care team, client, and family to coordinate care.
• Arrange family visits, transportation, and meet client needs.
• Maintain clear communication between family, referral sources, and Path Behavioral Healthcare.
• Attend court hearings as appropriate.
• Ensure documentation and billing adhere to policy and EHR standards.
• Monitor compliance with chart/documentation audits and billing reviews.
• Attend team meetings and present accurate case management reports.
• Facilitate individual and group meetings and participate in quality assurance activities.
