Job summary

CVS Health is seeking a Case Manager to support members enrolled in Medicare and Medicaid, focusing on complex health and social needs. You will coordinate care with providers, families, and community resources to promote wellness and effective outcomes.

Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Proficient with standard corporate software including MS Word, Excel, Outlook and PowerPoint.

• Assess member needs and eligibility and coordinate care plans.
• Use clinical judgment to reduce risk and improve outcomes.
• Provide crisis support and connect to appropriate providers.
