Job summary

CVS Health is seeking a full-time Case Manager to support dual eligible beneficiaries, coordinating care across Medicare and Medicaid services. You will conduct assessments, develop plans, and collaborate with providers to achieve quality outcomes while navigating multiple systems.

Applicants should be Illinois residents with CM, discharge planning, or home health experience, and be ready to travel. A Master’s in Behavioral/Mental Health and licensure (LCSW/LCPC) are preferred.

Qualifications

• Must reside in Illinois.
• 3-5 years clinical practical experience.
• 2-3 years CM, discharge planning and/or home health care coordination experience.
• Willing to travel up to 50-75% in specified areas.
• Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint).
• Strong analytical, communication and problem-solving skills.

Responsibilities

• Assess member needs and eligibility and coordinate care.
• Develop and monitor plans of care with care teams.
• Provide crisis intervention and referrals as needed.
• Collaborate with providers to improve health outcomes.
• Document care coordination activities per policy.
