Responsibilities

• Assess member needs and eligibility and develop care plans.
• Coordinate with supervisors and Medical Directors to address barriers to goals.
• Provide crisis intervention and refer to clinicians as needed.
• Document care and monitor plans of care in compliance with guidelines.
