Job summary

Community Partners NH in Strafford County seeks a Case Management Supervisor to oversee Behavioral Health Case Management. The role includes supervising staff, coordinating admissions-line support, and linking clients with community services to help them live independently.

The position requires a Bachelor’s Degree in human services or related field, case-management experience, strong communication skills, and exceptional attention to detail.

Qualifications

• Bachelor's Degree required in human services or related field such as Social Work, Psychology, or equivalent experience.
• Social work or case management experience in aging, disabilities, community health, nursing home or hospital discharge planning preferred.
• Experience in customer relations, call management, information and referral or related preferred.
• Excellent oral and written communication skills.
• Exceptional attention to detail.

Responsibilities

• Behavioral Health Case Manager provides phone support on the Agency’s Admissions Line and serves as a part of the Community Resource Center Department providing targeted case management to the clients for Adult Services, Youth and Family, or both teams.
• The Case Manager links clients with appropriate community service providers and organizations that empower individuals to live independently within their community.
