Community Partners NH in Strafford County seeks a Case Management Supervisor to oversee Behavioral Health Case Management. The role includes supervising staff, coordinating admissions-line support, and linking clients with community services to help them live independently.

The position requires a Bachelor’s Degree in human services or related field, case-management experience, strong communication skills, and exceptional attention to detail.

Qualifications

• Bachelor's Degree required in human services or related field such as Social Work, Psychology, or equivalent experience.
• Social work or case management experience in aging, disabilities, community health, nursing home or hospital discharge planning preferred.
• Experience in customer relations, call management, information and referral or related preferred.
• Excellent oral and written communication skills.
• Exceptional attention to detail.

What We’re Looking For

Bachelor's Degree required in human services or related field such as Social Work, Psychology, or equivalent experience.

Social work or case management experience, in the areas of aging, disabilities, community health, nursing home or hospital discharge planning preferred.

Experience in customer relations, call management, information and referral or related preferred.

Excellent oral and written communication skills.

Exceptional attention to detail.
