Milestone Senior Services seeks a BEHAVIORAL HEALTH CASE MANAGER to provide community-based mental health case management for adults 55+. Primary responsibilities include coordinating BH and NHM services, discharge planning, and advocacy to improve outcomes in the community.

The role requires a Bachelor’s degree in a human service field and relevant experience, with preferred MI licensure. The position offers an exempt, full-time schedule with competitive salary and comprehensive benefits,

Qualifications

• Bachelor’s degree in a human service field plus one year of mental illness experience.
• Preferred Michigan licensure (RN, LLBSW, LBSW, LLMSW, LMSW, LLPC, LPC, LLP, LP).
• Experience with public mental health system and elderly population preferred.

Responsibilities

• Manage BH and NHM caseload and provide community-based services.
• Coordinate with medical and ISK staff for discharge planning and safe return to the community.
• Educate and support families about needs and involvement and care planning.
• Complete timely documentation per funders and agency policies and procedures.
