Responsibilities

• Assist clients with releases of information to support care plans.
• Advocate for clients when there is a problem in service delivery.
• Provide crisis intervention as needed and help plan care with clinicians.
• Maintain assigned caseload and client contacts per contract/programs.
• Visit clients in homes and community to assess needs and services.
• Keep accurate, up-to-date records per standards.
• Deliver oral presentations about the mental health services program.
