Job summary

Socket.dev is seeking a Program Coordinator to provide community-based mental health and substance use services. You will assess needs, coordinate care, and support ongoing recovery for youth and adults, under supervision of the Program Coordinator.

You will deliver crisis support, day treatment services, and ensure documentation complies with HIPAA and 42 CFR. This role emphasizes patient engagement and collaboration with treatment teams.

Qualifications

• Bachelor degree in psychology, social work, or related field required.
• Experience with case management and mental health services a plus.
• Familiarity with HIPAA and clinical documentation.

Responsibilities

• Under the supervision of the Program Coordinator, provides community-based services to youth and adults with mental health and substance use disorders.
• Provides the scope of mental health and substance use related case management services including ongoing assessment of needs, skills for independence, coordination of ITP, linkage with supports, symptom monitoring, advocacy and outreach.
• Provides crisis support and stabilization activities including de-escalation, assisting with hospitalizations when indicated, safety planning, and coordination with the treatment team.
• Provides Day Treatment services to adults as assigned by Program Coordinator.
• Renders all services within department guidelines and in compliance with Medicaid/Medicare, HFAP, OhioMHAS, and CARF standards.
• Demonstrates commitment to quality care through professional growth and agency learning opportunities; uses best practices for patient engagement and retention during care episodes.
• Performs tasks to support revenue including documentation within guidelines; maintains treatment plans; handles grants or special projects as assigned.
• Effectively utilizes EMR and video-conferencing software; uses work email for department communications; ensures PHI compliance with HIPAA and 42 CFR laws.
